Health Provider Portal
You can access your patient's health records electronically by using the Health Provider Portal. If you don't already have access to the portal, you'll need to register.
Records include:
- discharge summaries
- medical imaging reports
- medication list
- outpatient letters
- operation reports
- pathology reports
- test results.
You may also be able to find the information you need on My Health Record.
Release of Information Unit
You can also request access to patient information from our Release of Information Unit.
You'll need to be specific about the information you need to care for your patient. For example, a copy of their colonoscopy report in 2018. Make sure you include your patient’s name, date of birth and consent to access their records.
We won't action a general request asking for a copy of a complete file. You'll be asked to refine the scope of your request.
